As such, we strive to comply with the Data Privacy Act of 2012 to secure your right to privacy and ensure the trustworthiness of processing your personal information.
In line with this, we intend to adhere to the principles set forth in this Privacy Statement to provide protection for your privacy. Purpose
Republic Act No. 11525 known as “An Act Establishing the COVID-19 Vaccination Program or The Philippine National Deployment and Vaccination Plan require collection of your personal information for Covid-19 Vaccination.
The declared purpose of the processing of personal data is to establish a masterlist of eligible population for vaccination to enable the LGU to decide on who can be safely vaccinated from a clinical perspective and other related purpose. Personal data collected for shall not be further processed for other purposes, which are incompatible with the specified and declared purpose.
Disclosure of your Personal Information
- to process their vaccination profiling form for vaccination schedule;
- monitor vaccination coverage and effectiveness;
- collect, analyse and publish statistics and other information about vaccination.
- advise an individual when they are, or were, due to receive a vaccine
- provide evidence of vaccination status;
- Provide DOH and other National Government Agencies any information or report, if needed or as required by law enforcement, government officials, or other third parties pursuant to a subpoena, court order, or other legal process or requirement applicable to our Agency.
- Provide third party contractors and/or sub processors that may have access to our databases in relation to the fulfillment of the declared purpose.
Any use and disclose personal information contained in the database shall be in accordance with the pertinent sections of the Data Privacy Act. Specifically the LGU reserves its right to process information that is necessary to protect vitally important interests of the data subject, including life and health or to respond to national emergency, to comply with the requirements of public order and safety, or to fulfill functions of public authority which necessarily includes the processing of personal data for the fulfillment of its mandate

The LGU is committed in ensuring that your information is secure. In order to prevent unauthorized access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect.
Limiting Use, Disclosure, Retention
The LGU identifies the purposes for which the information is being collected before or at the time of collection. The collection of your personal information will be limited to that which is needed for the purposes identified by us. Unless you consent or we are required by law, we will only use the information for the purposes for which it was collected. If we will be processing your personal data for another purpose later on, we will seek your further legal permission or consent; except where the other purpose is compatible with the original purpose. We will keep your personal data only as long as required to serve those purposes. We will also retain and use your personal data for as long as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements.
How we collect data from you
Personal data primarily may be collected upon filling up the online form provided in this site, or your submission of your personal data to us for availing of the Covid 19 Vaccine Registration, or any other reason including the submission of any records and official documents via electronic or physical means.
Storage and disposal of data
The LGU stores personal data in a database center (on premise and cloud) and physical document storage facilities.
It retains personal data only according to operational need and in compliance with legal and regulatory purposes. In general, the LGU shall only retain your data for five (5) years after the processing relevant to the purpose has been terminated. However, the LGU may retain your data when necessary to establish, exercise or defend legal claims, for legitimate business purposes, or when provided by law.
Ensuring the completeness, accuracy and protection of your personal data
You should ensure that personal data submitted to us is complete, accurate, true and correct. Failure on your part to do so may result in our inability to provide you with your vaccination schedule. You should inform the LGU immediately of any change of facts or circumstances which may render any information or personal data previously provided inaccurate, untrue, or incorrect and provide any information or documentation LGU may reasonably require for the purposes of verifying the accuracy of the updated information or personal data.
Additional Rights (RA 10173 Data Privacy Act of 2012)
- Right of erasure or blocking. You may have a broader right to erasure of personal data that we hold about you.
- Right to object. You may have the right to request that we stop processing your personal data and/or to stop sending you marketing communications.
- Right to restrict processing. You may have the right to request that we restrict processing of your personal data in certain circumstances.
- Right to access. In certain circumstances, you may have the right to be provided with your personal data in a structured, machine readable and commonly used format and to request that we transfer the personal data to another data controller without hindrance.
- If you would like to exercise any of the above rights, please contact our support team or contact our Data Protection Officer. We will consider your request in accordance with applicable laws. To protect your privacy and security, we may take steps to verify your identity before complying with the request.
- You also have the right to complain to a data protection authority about our collection and use of your personal data.
